EZ-IO® Hero Entry Form

Entry Information & Rules:

Vidacare has launched the Hero Campaign to recognize medical professionals who have used the EZ-IO to quickly and safely gain vascular access in emergency situations. Simply fill out the form below describing the Nurse, Paramedic or Physician you witnessed using the EZ-IO and why you feel they should be recognized as Vidacare’s Hero of the quarter. Voting will take place on a quarterly basis. The quarterly winners will receive a recognition plaque. Voting will take place at the end of the year for a grand prize winner who will be recognized at EMS Today in March 2010 and receive a $2,000 educational grant to award in their community.

  • Heroes will be nominated and selected on a quarterly basis
  • A person cannot nominate himself or herself
  • The winning entry must be based on the use of the EZ-IO to gain vascular access
  • The nominee must have been affiliated with an emergency service or hospital at the time of the event
  • The nominee must be currently affiliated with an emergency service or hospital
  • Winners must agree to have their story used in Vidacare promotional materials

Vidacare’s process involves reviewing all stories submitted to verify compliance with label usage requirements prior to publishing for consideration.
